What is this form used for?
---------------------------

  This form is for use by organisations requesting an Autonomous System 
  (AS) number used to enable routing in multihomed environments.
  
  It may be used by APNIC members and non-members with a valid
  APNIC account name.
  
  If your organisation is requesting an AS number on behalf of a
  customer, please be aware that your organisation must return the AS
  number to APNIC if the customer ceases connectivity with you.

Eligibility for an AS number
----------------------------

  Organisations requesting an AS number should see RFC 1930 to 
  determine whether an AS number is justified. Note that 
  organisations who connect directly to a public Internet exchange
  point are regarded as being multihomed.

  AS numbers are available to both current APNIC members and non-
  members. Non-members must first obtain an APNIC account name.

APNIC assigns four-byte AS numbers by default. 
If you require a two-byte AS number, please provide the technical 
reason(s) for this requirement.

If you are an APNIC member, are you requesting this AS number on
behalf of a customer?

customer ceases to receive connectivity from you?
